Re: Complaint made by Malcolm Baker.
Regretfully I cannot recollect Mr Baker or his stay in the Intensive Psychiatric Care Unit between 6.7.92 and 11.7.92. Given the vast numbers of patients that pass through the unit in the course of a year, (well in excess of six hundred in the last year); this is not too surprising. Consequently, I have been forced to rely on the existing documentation in Mr Baker's historical file, included in this review were the nursing notes, medication and drug administration cards, the seclusion forms and checklists.
